The Health Coach RN provides lifestyle health coaching with behavior modification to assist members in health self‑management for a better quality of life. The role relies on education, experience and judgment to plan and help members accomplish health goals. Creativity and latitude are expected to complete the responsibilities, performing a variety of tasks under general supervision. Responsibilities Individual Risk Coaching Assists employer health plan members to set realistic holistic goals using system tools and judgement, gaining trust to assure disease‑based, risk or high‑utilization individuals set and meet or exceed targets. Consistently measures and communicates with the individual on goal status and documents progress using system tools. Processes include constant communication, psycho‑social skills, and disease‑management care plans such as obesity, diabetes and hyperlipidemia. Coordinates care team around the individual to assure obstacles to goals are removed, including making appointments, ensuring prescriptions are filled, and directing care to physicians, specialists, EAP or other providers. Participates in member Healthy Lives screenings to obtain biometric data, documents biometric data appropriately in the online tool, and provides brief coaching based on biometrics obtained. Communication and Collaboration Communicates effectively with the provider to assure the panel of individuals is on track with health goals. Ensures care plans in the health coaching tool align with the provider’s treatment, including outcomes for measurement of disease, wellness and other goals. Participates in Healthy Lives screenings to obtain member biometric data and documents it in the online tool. Assists with worksite assessments, evaluating policies for vending, catering and dining as needed. Produces and participates in the creation of wellness webinars, newsletters, presentation content and other special projects as needed. Makes referrals to and receives referrals from Care Management, In‑Home Monitoring and RN Care Coordinator as needed. Employer Satisfaction Documents progress to goals in the health coaching tool for employer reports on compliance with wellness screening recommendations. Estimates fact‑based savings for individuals to report cost savings to employers due to weight loss and a holistic approach to personalized health coaching. Qualifications 2 years of related experience. Education See licensure requirements. Special Skills Excellent written, verbal and presentational communication skills.
